Your Hip May Not Be the Problem

You’ve been told it’s bursitis.

Or sciatica.

Or arthritis.

You’ve completed physical therapy, tried medications, maybe even had injections, yet walking, sitting, climbing stairs, or sleeping on your side still hurts.

For many patients, the real question isn’t “What treatment should I try next?”

It’s “Has anyone identified what’s actually causing my pain?”

At Alypos Regenerative Pain & Wellness, the focus is on finding the true source of your symptoms so treatment can be directed with precision—and support healing rather than simply provide temporary relief.

A Different Starting Point

Sacroiliac and pelvic pain are frequently mistaken for generalized lower back pain, hip pathology, or lumbar radiculopathy.

At Alypos, evaluation focuses on movement asymmetry, ligamentous support, referred pain patterns, nerve involvement, prior treatment response, and real-time ultrasound assessment to identify potential pain generators and procedural targets.

Treatment decisions are guided by diagnosis, anatomy, and clinical judgment—not a one-size-fits-all protocol.

Common Reasons Patients Come to Alypos

My hip MRI looks normal, but I still hurt

Imaging doesn’t always explain the source of pain.

I’ve had injections, but the pain keeps coming back

Temporary improvement may mean the underlying problem wasn’t fully addressed.

My pain is in my buttock, not my joint

Pain may arise from the sacroiliac joint, gluteal tendons, ligaments, nerves, or nearby structures.

Everyone tells me it’s sciatica

Not all buttock or leg pain comes from the spine.

Physical therapy helped, but I plateaued

Persistent pain deserves a closer look.

No one explains why I still hurt

A fresh evaluation can identify findings that have been overlooked.

Why Hip and Buttock Pain Is Often Missed

Pain around the hip and pelvis is complex.

The hip joint, sacroiliac joint, gluteal tendons, ligaments, muscles, peripheral nerves, and lower back can all produce similar symptoms.

As a result, many patients receive treatments aimed at the location of the pain rather than the structure responsible for it.

At Alypos, the evaluation integrates your history, examination, prior imaging, movement patterns, and dynamic ultrasound when appropriate to determine what is actually causing your symptoms.

Treatment follows the findings—not a one-size-fits-all protocol.

Alypos Is Different

Many patients who seek care at Alypos have already received treatment elsewhere.

The difference is not simply the procedure—it’s the evaluation that comes before it.

Every consultation and procedure is personally performed by Dr. Bernhardt, a triple board-certified physician with more than 20 years of experience in image-guided procedures and regenerative medicine.

Records, imaging, examination findings, and real-time ultrasound are combined to identify the true source of symptoms and guide treatment with precision.

Alypos  was designed for patients who want thoughtful analysis, technical precision, and treatment tailored to the structure actually responsible for their pain.

Regenerative Medicine Is Not a Commodity

Not all PRP or regenerative products and procedures are the same.

Success depends on selecting the right patient, identifying the correct tissue, and delivering treatment accurately under image guidance. Physician expertise matters.

Regenerative therapies are used to support healing in tendons, ligaments, joints, and other musculoskeletal structures with the goal of improving function—not simply masking pain.

Ready for a Different Approach?

If you’ve been living with persistent hip, buttock, or sacroiliac pain despite previous treatment, it may be time for a more focused evaluation.

The first step isn’t another generic injection.

It’s understanding what’s actually causing your pain—and whether a more precise strategy can help.

Frequently Asked Questions

My hip MRI is normal. Why do I still have pain?

Hip and buttock pain may arise from the sacroiliac joint, gluteal tendons, ligaments, muscles, peripheral nerves, or referred pain patterns that are not always obvious on MRI. A normal scan does not necessarily mean nothing is wrong.

Could my pain actually be coming from my sacroiliac (SI) joint?

Yes. Sacroiliac joint dysfunction is a common cause of lower back, buttock, and even leg pain. It is frequently mistaken for lumbar spine disease or hip pathology and often requires a careful examination and targeted evaluation to diagnose.

What is greater trochanteric pain syndrome?

This term describes pain over the outside of the hip and often involves irritation of the gluteal tendons or surrounding soft tissues. It is commonly mislabeled as “hip bursitis” and may require a different treatment approach.

Can gluteal tendons be injured or degenerate like the rotator cuff?

Yes. The gluteal tendons can become injured or degenerative over time, leading to pain with walking, climbing stairs, or lying on the affected side. These conditions may respond differently than simple inflammation.

I've already had injections. Why didn't they last?

Temporary improvement may mean the correct structure was only partially treated, another pain source was present, or the treatment addressed symptoms without supporting tissue healing.

Can regenerative medicine help hip or sacroiliac pain?

For carefully selected patients, regenerative treatments such as platelet-rich plasma (PRP) and Stem Cells may be used to support healing in injured tendons, ligaments, joints, and other musculoskeletal tissues. Suitability depends on the diagnosis and examination findings.

Is all PRP or regenerative medicine the same?

No. Outcomes depend on accurate diagnosis, patient selection, image-guided precision, biologic preparation, and physician expertise. Regenerative medicine is not a commodity, and the quality of the evaluation and procedure matters.

Do you only treat the hip joint?

No. Evaluation may include the hip, sacroiliac joint, gluteal tendons, surrounding ligaments, muscles, and other structures that can produce similar symptoms. The goal is to identify the true source of your pain rather than assume it comes from the hip itself.

Who performs the evaluation and procedures?

Every consultation and procedure at Alypos is personally performed by Dr. Brian Bernhardt, a triple board-certified physician with extensive experience in image-guided pain and regenerative medicine.

Do you accept insurance or Medicare?

Alypos Regenerative Pain & Wellness is a physician-only, direct-pay practice and does not bill insurance or Medicare. This model allows treatment decisions to be based on your clinical needs rather than insurance restrictions.

Will I know the cost before treatment?

Yes. Fees are discussed in advance so you can make informed decisions without surprise bills or facility fees.

Can I use my HSA or FSA?

In many cases, HSA and FSA funds may be used for qualified medical expenses. We recommend confirming eligibility with your plan administrator.
