Physician-only care
Thoughtful. Precise

Brian Bernhardt, MD

For Patients whose pain has not responded to standard treatment—and for those who want a more thoughtful approach earlier—Dr. Bernhardt focuses on understanding why pain persists and what approach is most appropriate for the individual patient.

Meet Dr. B

Dr. Brian Bernhardt is a triple board certified anesthesiologist and interventional pain physician with expertise in advanced regenerative therapies, and comprehensive pain modulation.

Grounded in detailed anatomy, real time diagnostics (including high volume ultrasound, and fluoroscopy), and individualized clinical judgment, he founded Alypos Regenerative Pain & Wellness—a physician -only practice created for patient seeking discretion, expertise, and truly personalized care beyond conventional models.

A Physician-Only Practice

Alypos was intentionally designed outside the conventional high-volume medical model. Every patient is personally evaluated, treated, and followed by Dr. Bernhardt.

There is no delegation of procedures, mid-level substitution, or protocol-driven care applied without individual clinical justification—ensuring precision across spine, peripheral joints, soft tissues, and regenerative applications.

Unconventional Path and Formative Experiences

Before medicine, Dr. Bernhardt’s path was unconventional. As a young adult, he competed as a rodeo bucking horse rider—an experience that exposed him early to significant physical injury and recovery. This shaped his profound respect for pain, function, and the long-term consequences of musculoskeletal issues.

As a young man, Dr. Bernhardt was exposed to traditional Indigenous healing traditions through time spent with respected elders, including members of the Sioux and Karuk communities. These formative experiences emphasized observation, restraint, and respect for the body’s capacity to heal—principles that continue to influence his disciplined, patient-centered approach within modern medicine.

Early exposure to traditional healing philosophies reinforced a principle that remains central to Alypos today: meaningful recovery requires respect for the whole patient, not just the symptomatic structure.

That early immersion emphasized observation, restraint, and respect for the body’s capacity to heal—principles that contrasted sharply with the rigid, protocol-driven systems he later encountered in conventional medicine.

Training and Clinical Insights

After earning his GED, Dr. Bernhardt progressed through community college and scholarship to the University of Southern California, followed by his medical degree. He completed postgraduate training in obstetrics and gynecology, anesthesiology, and advanced interventional pain management. He is board certified in Anesthesiology, Pain Medicine, and Interventional Pain.

His career spans the opioid era’s challenges, where he observed systemic limitations in addressing complex pain pathology—often prioritizing symptoms over root causes, with barriers to regenerative options. These experiences, combined with the personal impact of patient outcomes, reinforced his commitment to time-intensive, judgment-driven care that bridges what medicine can achieve with what patients truly need.

Clinical Philosophy

Dr. Bernhardt’s approach is highly selective and evidence-informed, favoring interventions that promote tissue health, durable function, and long-term outcomes over temporary symptom relief. With personal experience undergoing many of the procedures he performs, he applies regenerative and interventional techniques with restraint.

At Alypos, care extends beyond traditional "pain management" and encompasses the full spectrum of musculoskeletal and functional conditions, including axial spine disorders, peripheral joint pathology, tendon and ligament disorders, neuopathic conditions, and select regenerative applications—always individualized and physician-delivered.

This philosophy drives a practice rooted in discipline, direct accountability, precision diagnostics and interventions— prioritizing lasting results over volume or expediency.

Clinical Leadership and Contributions

Dr. Bernhardt has trained under nationally recognized leaders in pain and interventional medicine. He contributes to the field through peer-reviewed publications, textbook chapters, and leadership roles in interventional practices—overseeing operations, educating physicians and trainees, and serving as a medical expert/consultant providing analysis and testimony in complex musculoskeletal and pain-related cases. With over a decade in expert consultation and high-volume procedural experience—including advanced ultrasound and flouroscopic-guided interventions—he brings authoritative insight.

The Alypos Standard

The Alypos standard reflects a disciplined commitment to clinical judgment, direct physician accountability and care designed to achieve durable outcomes-not expedient solutions.

Alypos serves patients seeking thoughtful, physician-provided care delivered with precision, transparency and respect for time and judgement.

Alypos was built for patients who value care informed by experience, judgment, and an understanding of pain from both sides of the exam table.
